Student Referral

How to refer a student to the Assistive Technology Unit:

1. Print the Assistive Technology Referral Form from Inside Edition or request one by calling the OT / PT Department at (847) 599-4212.
2. Assign a contact person from student's educational team.
3. Complete forms using input from the student's entire educational team.
4. Mail all the forms to the OT / PT Department at WHS Washington Campus.
