Student Referral
How to refer a student to the Assistive Technology Unit:
-
Print the Assistive Technology Referral Form from
Inside Edition or request one by
calling the OT / PT Department at (847) 599-4212.
- Assign a contact person from student's educational team.
- Complete forms using input from the student's entire educational team.
- Mail all the forms to the OT / PT Department at WHS Washington Campus.
