A new request process has been implemented starting at the beginning of the 2007-08 school year.

All requests should be made through your building's "Problem Solving Team".  If your building does not have RTI established yet, the referral may be completed by the classroom teacher or case manager.    

Prior to filling out a Request for OT or PT Professional Consultation, the teacher &/or educational team should try basic interventions and record data on the results of the interventions.  If the inventions are not successful, then a "Request for Occupational or Physical Therapy Professional Consultation for Problem Solving" form should be completed (front & back sides).   The form should then be sent to the OT/PT Department at the WHS-Washington campus via the district's mail.

Please only place one discipline per request form.  If you want to make a request to both OT and PT, then please complete 2 separate forms (1 for OT & 1 for PT).
