The first plan is an indemnity PPO plan. There is a $500 deductible, co-pay of 80% after deductible and 100% after you have reached your out-of-pocket expense of $2,000. Those benefits are paid as long as you utilize a provider in the PPO. There are thousands of providers in the PPO. You can access the physicians on the plan by searching on the Internet at www.ppippo.com. If you do not utilize a PPO provider, benefits are paid at 70%. The PPO plan has a prescription card with a co-pay of $20 for brand name drugs and $8 for generic.

HMO Illinois is the second plan that the District offers. As with most HMO's, the employee would pick a primary care doctor and be referred to a specialist if needed. The HMO also offers a prescription card.

All new employees start out with the Waukegan Public School PPO Plan. They may switch plans during open enrollment in the month of September. A reminder notice will be sent to all employees reminding them of the open enrollment.

The District has one dental plan at the current time. Whether you chose the Waukegan Plan or the HMO you will have the Waukegan Public School's Dental Plan.
